When you complete the Ulta customer satisfaction survey, you are entered into the company’s sweepstakes. The Ulta company wants to ensure that you have a positive and uplifting shopping experience, so your honest feedback is extremely important to them. If you provide some basic personal information at the end of the survey, you will be entered in the monthly drawing to win a $500 gift card for a future Ulta purchase. Ulta Sweepstakes Rules
In order to be eligible for the sweepstakes, you must meet the following requirements:
- Be a legal resident of the United States
- Be at least 18 years of age
- Not work for Ulta or any of its affiliates or marketers
- Not be in the immediate family or household of an Ulta employee
- There are 12 monthly drawings and you may enter only once a month
- There is one winner per month and the company will only make two attempts to contact winners
- Winners are responsible for all taxes

Ulta Survey Steps
There are two ways to enter the sweepstakes.
- 1. Go online to survey.ulta.com.
- 2. Enter the store # from your product receipt.
- 3. Enter a valid email address (so that you can be contacted) and click “continue”.
- 4. Follow the instructions to answer all the questions and add your comments.
- 5. Provide your personal contact information to be entered into the sweepstakes.
- 1. Enter the sweepstakes without completing the survey.
- 2. On a 3” X 5” index card, print your name, address, and telephone number.
- 3. Place in an envelope (no larger than 4 1/8 X 9 ½ ) and mail via first class mail to Ulta Guest Satisfaction Entry, 1135 Arbor Drive, Romeoville, IL 60446.
