It’s fairly simple to enter the sweepstakes but there are some requirements you need to meet to be eligible.
- You must be a resident of Florida, Georgia, Alabama, South Carolina, North Carolina, or Tennessee.
- You must be at least 18 years of age.
- You must not be an employee of Publix or any of its affiliates, or a family member of an employee.
- You may only win one prize per household.

Here are the steps to follow to complete the survey and enter the sweepstakes:
1. Go to the Publix survey homepage.
2. Read the short description.
3. Choose either English or Spanish.
4. Enter the store number, survey code, and time stamp from your store receipt into the appropriate boxes and click “Next”.
5. Answer each question honestly.
6. Enter your personal contact information so that you may be contacted if you win the sweepstakes.
If your receipt has no survey code, check the box asking you to enter without a survey code. Enter store number and time stamp, and click “Next”. Enter your phone number and proceed to the questions.
You may also enter the sweepstakes if you have no receipt or haven’t made a purchase. Write your name, phone number and the location of your nearest Publix store on a letter or postcard. Mail it to Publix Customer Voice Survey Sweepstakes, Marketing/Research Department, 3300 Publix Corporate Parkway, Lakeland, FL 33811.
